A signed, authorized, and current affiliation agreement between UCSF Health and the academic institution must be in place prior to approval of all clinical placements. The existence of an affiliation agreement does not signify approval of clinical placement or practicum requests. If you are unsure about the status of an affiliation, please contact your school administrator.  

Note: As of April 2026, all requests for new or renewals of affiliation agreements are submitted to UCSF Procurement using the Oracle platform. The requisition form in the Oracle platform may be accessed through MyAccess (SSO) and must be submitted by the UCSF approver/ manager (Affiliate’s Sponsor).  Students or school representatives may not request initiations or renewals directly to UCSF Procurement. Requisitions must be submitted by the UCSF approver/manager on behalf of the student.

An affiliation agreement is typically valid for five years. If your clinical placement will extend beyond the agreement terminate date, please submit a new request as they do not automatically renew. Processing time may take up to three months.
