Affiliation Agreements

A signed, authorized, and current affiliation agreement between UCSF Health and the academic institution must be in place prior to approval of all clinical placements.  The existence of an affiliation agreement does not signify approval of a clinical placement or practicum request.  Please note it is the responsibility of the student or faculty designee to secure a UCSF Health preceptor prior to the start of the clinical practicum.

An electronic requisition form using DocuSign must be submitted to UCSF Procurement to initiate or renew an affiliation agreement. The person completing the form must be a UCSF employee, preferably the student’s preceptor. After the form is submitted, Procurement will contact the school to negotiate, draft, and execute an agreement.  A fully executed copy is then sent to the school and the requestor is notified.

An affiliation agreement is typically valid for five years. If your clinical placement will extend beyond the agreement terminate date, please submit a new request as they do not automatically renew. Processing time may take up to three months. If you are unsure about the status of a submitted requisition, please contact the contract officer at the student’s academic institution.
