Elise Hazlewood, MS, RN, CCNS, is the Cardiopulmonary Clinical Nurse Specialist at UCSF Helen Diller Medical Center at Parnassus Heights in San Francisco, CA. She supports nursing practice on the cardiovascular and thoracic transitional care units, provides multidisciplinary consultation for pulmonary hypertension (PH) patients and provides education for patients and caregivers. Elise is a known resource for PH nursing care, and facilitates ongoing PH nursing education. She previously worked as the PH nurse coordinator, providing inpatient coordination of care and facilitating the transition to outpatient care.
